Workshop #1: Team Based Care-Role of Pharmacists in Hypertension Control presented by The American Heart Association, Missouri Department of Health and Senior Services, and the Health Quality Innovation Network. Recommendations and updates presented from the Community Preventive Services Task Force (CPSTF), (The Community Guide), team-based care and the role of the Pharmacist in improving patients’ blood pressure. Evidence shows team-based care
increases the proportion of patients with controlled blood pressure. Pharmacists play an important role in Team Based Care from medication review, monitoring medication adherence and patient education on SMBP, diet and heart healthy living.
